ARRIVAL & CONSULTATION
Prior to your visit, we ask that you complete the New Patient Intake forms online. If you are unable to do so, please let us know and we will schedule time for you to complete the forms in-office.
Whenever possible, we like to verify benefits before your first visit. When you call to schedule your first appointment, have your insurance card handy. At your first appointment, we will take a photocopy of your insurance card and also your driver’s license or other state issued photo ID.
Next, you will have a consultation with the doctor to discuss your health related problems, concerns, and potential treatment options. This initial visit is designed so we may learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.
After your consultation, Dr. Holt or Dr. Tucci will perform a complete chiropractic examination. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. But remember, nothing will be done in our office without your consent.
REPORT OF FINDINGS
Once the information is collected and examinations are performed, you will receive a detailed report of all findings and answers to any questions including;
- How can you help me?
- How often do I need to come in?
- What will be my treatment cost?
After reviewing your health history and examination findings,
Dr. Holt or Dr. Tucci will discuss recommendations and notify
you if your condition requires care with other providers. Dr. Holt
or Dr. Tucci will provide the best treatment and wellness
program for your needs.
At the conclusion of this initial appointment, you can decide if you want to move forward with your first treatment. We strive to make the experience relaxed and comfortable at all times. You will be amazed how good you can feel!
If you are in pain when you first come into our office we may advise; ice or heat application instructions, certain activities or positions to avoid, and at-home exercises and/or stretches. Every person is unique and requires a customized treatment plan. Our only goal is to get you well and keep you that way!
